The following is for ATV safety program grantees:

Below you will find links to the midterm and final report forms. As part of your agreement to accept funding from National 4-H Council for your ATV safety program, you agreed to participate in the reporting process. The midterm and final reports are brief written statements that we use to check the status of your program and keep us up to date of any changes in your program. Please take a few minutes to fill out the forms and send them back to us.

The midterm report is due Monday, February 3, 2003, and the final report is due Tuesday, July 1, 2003. Please download the forms below and send them to via e-mail as an attachment.
